Children identified with a potential vision problem will be provided a packet containing the screening results, a referral letter, and contact details for eye care professionals. Families will be encouraged to seek further evaluation and treatment by eye care professionals and will be asked to take this Evaluation Sheet to their examination.

Will insurance or Medicaid cover follow-up exams if my child is referred?  Individual health insurance or Medicaid should cover most exams, however, plans will vary and you may need to contact your child's primary care provider.
