To receive a current application that can be filled out electronically, please call the Division of Continuing Medical Education at (319) 335-8599 and ask to speak to one of our program coordinators. We ask that you not type over your previous year's application because you may not be working with the most current document. The application must be filled out by the course director to receive approval.

Standards for Integrity and Independence in Accredited Continuing Education

All planners, faculty, and others in control of educational content are required to disclose all financial relationships with ineligible companies within the prior 24 months before planning the education.   An ineligible company is any entity whose primary business is producing, marketing, selling, re-selling, or distributing healthcare products used by or on patients.

The ACCME Standards for Integrity and Independence require that we disqualify individuals who refuse to provide this information from involvement in the planning and implementation of accredited continuing education.

Financial relationships must be reviewed to identify which relationships are relevant.  Relevant financial relationships must be mitigated prior to the individuals assuming their roles.  

UI Health Care Speakers & Planning Committee Members

  • Disclosure:  UI Health Care speakers/planning committee members should disclose through the Conflict of Interest Office's eCOI Database
  • Review:  Course Directors can view UI Health Care speakers' disclosure of financial relationships, and they can complete conflict of interest review reports at CME Division Disclosure and Conflict of Interest Review (contact the CME Office (5-8599) if you do not have access). Contact the CME Office for guidance in reviewing and mitigating financial realtionships.
