Asymptomatic left neck mass identified 12 months previously was associated with slight fluctuation in size leading to ultrasound evaluation with needle biposy showing 'atypical cells' warranting gland resection.

Left submandibular gland resection identifies that the deep portion of the gland near the ductal drainage was normal on histopathology with the superficial portion of the gland (further from the ductal drainage) showing chronic inflamation and fibrosis.
