Children identified with a potential vision problem will be provided with a list of local eye care professionals for evaluation and possible treatment. Families will be encouraged to seek further evaluation and treatment by local eye doctors and will be asked to take this Evaluation Sheet to their examination.

Will insurance or Medicaid cover follow-up exams if my child is referred?  Individual health insurance or Medicaid should cover most exams, however, plans vary and you may need to contact your child's primary care provider.
