Laryngocele (saccular cyst) with histopathology (laryngocoele)

History: 55 yo man with long smoking history and new sensation of obstructing 'mucus-like' feeling in his throat for 6 months with altered voicing

Modified Operative Note:

Preop Dx: Left saccular cyst, dysphonia

Postop Dx: Same

Procedure: Microdirect laryngoscopy with resection of left false vocal cord and saccular cyst (equivalent to an endoscopic supraglottic partial laryngectomy)

Anesthesia: General OETT - glides scope identifying obstructing lesion bypassed by endotracheal tube - no problems encountered in mask ventilation

Findings: as per photos - entire saccular cyst resected grossly with preservation of pyriform sinus/ A-E fold, true vocal cord

Use of CO2 laser 4 then 6 watts continuous superpulse with cautery (hand held to suction and to endoscopic forceps) for control of small vessel bleeding
