Chronically enlarged parotid glands in patient with diabetes and a BMI = 45 is consistent with sialosis based on clinical presentation and radiographic imaging. Sialosis may be accompanied with pain or may be painless.

Enlarged parotid glands bilaterally (white arrows show right) with no sign of ductal dilation, stone, obstruction or inflammation.

Right parotid sialogram confirms integrity of ductal drainage system showing enlarged gland with splaying appart of acini c/w sialosis.
