Also known as thyroglossal duct remnant
- Most common congenital tongue base and neck lesion
- 7% on autopsy will be found to have cyst
- Rarely associated with thyroid carcinoma of the cyst
- Found between the foramen cecum of the tongue base and the infrahyoid thyroid bed
- A midline cystic neck mass embedded in the infrahyoid strap muscles
- Almost 50% are found at the level of the hyoid, with half of the remaining found above and half below
- Usually 2-4 cm in diameter but can be smaller
- On CT: benign appearing, low-density, occasionally septated, thin rim of peripheral enhancement with contrast, thicker rim if infected
- On MR: can be hypo- or hyperintense on T1 depending on protein content; hyperintense on T2; rim-enhancing on T1 post-contrast if infected
