VA dictation instructions

  1. Call: inside hospital – 3200, outside – 887-4970
  2. At prompt, enter site ID: 10, followed by # key. Then ID number and #. Pass code (is ID in reverse followed by two zeros) and #.
  3. Enter work type (2- OP report, 3- discharge summary, 9- non-OR procedure)
  4. Enter patient ID (SSN) and #
  5. At prompt “ready to dictate”, press 2 to begin recording (or *13 to listen). See UIHC dictation instructions for keypad commands

  1. H&P update and mark patient
    1. If cannot mark (eg. DL), place wristband with patient's name, procedure name, and your initials
  2. Surgeon's op note

  4. If outpatient

    2. 8W order set
    3. Discharge order
    4. Prescriptions
      1. Note: no liquid pain medications; paper Rx required for all narcotics except Lortab
  5. If inpatient
    1. Delayed admission orders
    2. Admission / post-op check note
