- Curriculum vitae
- Three letters of reference
- Two sample case reports
- Graduate school transcripts

Information for Reference / LOR Providers
Reference Provider Portal
After the applicant enters your information (i.e. email address) as a reference provider, you will be sent an email with instructions on how to submit the letter of recommendation/reference letter using the portal. If the reference providers email server blocks the message, please try an alternate private mail address, it does not have to be an institutional address.

Other material that the candidate would like the Program Selection Committee to consider is welcome. Early application is strongly encouraged, and applications are due by December 1, 2024.
Applicants are notified of their status vis-à-vis our program well in advance of the February meeting of the International Neuropsychological Society, and arrangements are made to interview with our faculty at the INS meeting. Typically, we interview highly ranked applicants at INS, although alternative interview formats (phone interview, Skype, in-person interview) may also be utilized.
Our Program participates in the APPCN Match Program. The APPCN match number for our Program is 9812. We rank all competitive applicants. Applicants are provided feedback about their status vis-à-vis our program in accord with APPCN guidelines. The match process ultimately dictates which applicant(s) are accepted to our Program.
