Research Spotlight: During critical BCG shortage, UI Urology study of chemotherapy combination for non-muscle invasive bladder cancer (NMIBC) offers possibility of a viable alternative for bladder salvage

Led by Dr. Michael A. O’Donnell, director of urologic oncology, and Dr. Vignesh T. Packiam, the UI Urology team in 2022 published multiple studies documenting the use of sequential intravesical chemotherapy combinations in the treatment of patients with NMIBC, most notably a combination of gemcitabine and docetaxel (gem/doce) that has all but replaced BCG, an immunotherapy that has been the standard of care for NMIBC for nearly five decades, as first-line therapy for NMIBC patients at UI Hospitals & Clinics since Dr. O’Donnell invented the therapy nearly 15 years ago.

A study led by Dr. Packiam and published in the September issue of the Journal of Urology was a highlight. It was the largest study to date of the use of gem/doce in BCG-naïve patients with high-risk disease.

The authors conducted a retrospective study of 107 BCG-naïve patients with high-risk disease treated at UI Hospitals & Clinics from from 2013 to 2021. The analysis revealed that 82% of those patients remained cancer-free two years after treatment. Less than 4% of those patients were unable to tolerate a complete course of treatment. Historically, two-year cancer-free rates in patients treated with BCG hover between 60 and 65%

In the NCI Cancer Currents blogUrology Times, and other outlets, Dr. Packiam hailed the findings as a milestone in the search for a viable replacement for BCG.

Urologic oncologists around the country appear to agree. In a June article in AUA News co-authored by Dr. Packiam, the authors reported the results of a survey of North American urologic oncologists in which 71% of the 198 respondents confirmed that they had prescribed gem/doce for NMIBC in the previous year. The authors estimated that at least 1,300 NMIBC patients now receive the UI-pioneered regimen annually.

Through 2022, the study of gem/doce and other chemotherapy combinations was limited almost entirely to retrospective studies, with the bulk of the data coming from UI Hospitals & Clinics cases. However, a large, multi-center, randomized prospective study conducted by the ECOG-ACRIN Cancer Research Group comparing gem/doce and BCG will begin enrolling patients this year. Drs. O’Donnell and Packiam were key contributors to the study’s design, and Dr. Packiam will serve as the UI’s principal investigator and as an advocate for the trial.
